Referrals Process

We operate a clear, responsive referral pathway designed to support timely placements while ensuring safety, compatibility, and positive outcomes for individuals.

1) Initial Enquiry (24–48 hour response)
Referrals are accepted from local authorities, NHS partners, and housing teams via phone or email. We provide early feedback on likely suitability and availability to avoid unnecessary delay.

2) Referral Information Review
We request core information (needs, risks, safeguarding history, outcomes sought). This enables proportionate

decision-making and safe matching.

3) Compatibility & Risk Assessment
Each referral is assessed against:

  • The individual’s needs and outcomes

  • Existing residents (where applicable)

  • Environmental and staffing capability

  • Safeguarding and risk management requirements

4) Pre-Placement Meeting (where appropriate)
We meet the individual and professionals to confirm expectations, reduce placement breakdown risk, and agree transition arrangements.

5) Decision & Offer of Placement
A clear accept/decline decision is provided promptly, with reasons. Where accepted, we confirm start date, support model, staffing, and contractual arrangements.

6) Transition, Review & Outcomes Monitoring
We agree a transition plan and early review points. Outcomes are monitored and shared with commissioners through regular updates and reviews.
